ACS Homecare helps eligible Pennsylvanians access state-funded in-home care through the Community HealthChoices (CHC) and Medicaid Waiver programs — so you or your loved one can remain safely at home.
What Is Community HealthChoices?
Community HealthChoices (CHC) is Pennsylvania's mandatory managed care program for adults who are eligible for homecare services through Medicaid.
ACS Homecare works directly with CHC managed care organizations (MCOs) and Medicaid Waiver programs to provide personal care, homemaker services, and companion care to eligible participants — at no cost to the individual.

Services We Provide
Personal Care
Assistance with bathing, grooming, dressing, toileting, and other activities of daily living (ADLs) to support independence at home.
Homemaker Services
Light housekeeping, laundry, and meal preparation to maintain a safe and comfortable home environment.
Companion Care
Friendly companionship, supervision, and social engagement to reduce isolation and support emotional well-being.
Respite Care
Temporary relief for family caregivers, giving them a break while ensuring their loved one continues to receive quality care.
Care Coordination
Our dedicated Care Coordinators work with MCOs and families to develop and manage individualized care plans.
Medication Reminders
Reminders to take prescribed medications on schedule, supporting health management and safety at home.
Who Qualifies?
You may be eligible for CHC or Medicaid Waiver home care services if you meet the following criteria:
- Pennsylvania resident aged 21 or older
- Enrolled in Medicaid (Medical Assistance) or dually eligible for Medicare and Medicaid
- Require a nursing facility level of care as determined by an assessment
- Prefer to receive care at home rather than in a facility

ACS Homecare assigns a qualified, compassionate caregiver and your ACS Care Coordinator manages your case from day one.
Managed Care Organizations We Work With
ACS Homecare is an approved provider with Community HealthChoices managed care organizations serving southeastern Pennsylvania. We work with your MCO to coordinate services seamlessly.
