Community HealthChoices (CHC) / Pennsylvania Waiver Program

What is Community HealthChoices (CHC)?

Community HealthChoices (CHC) is Pennsylvania's Medicaid managed care program that helps older adults and individuals with physical disabilities receive long-term services and supports in their homes and communities instead of a nursing facility whenever possible. The program coordinates medical care, prescription coverage, and home care services to help participants
remain safe, independent, and comfortable at home.

At ACS Homecare, we help eligible participants receive high-quality in-home care through the CHC Waiver Program. Our caregivers provide personalized assistance that allows individuals to continue living in the setting they prefer—their own home.

Who May Qualify?

You may be eligible for Community HealthChoices if you:

  • Are age 21 or older and have a physical disability, OR are age 65 or older
  • Qualify for Pennsylvania Medical Assistance (Medicaid)
  • Require a nursing facility level of care but can safely remain at home with support services
  • Meet the financial and functional eligibility requirements established by the Pennsylvania Department of Human Services (DHS)

Depending on individual needs and assessment results, CHC participants may receive services such as:

  • Personal care (Bathing, grooming, and dressing assistance)
  • Mobility and transfer assistance
  • Toileting and continence care
  • Meal preparation and feeding assistance
  • Medication reminders
  • Light housekeeping
  • Laundry and linen changes
  • Grocery and pharmacy assistance
  • Accompaniment to appointments
  • Assistance with coordinating transportation

Services are determined through an individualized care plan developed by the participant,their Medicaid Service Coordinator and ACS Care Coordinator.

Can a Family Member Be Paid to Provide Care?

Yes. Through participant-directed services, many CHC participants can choose a family member or trusted individual to provide their care. Pennsylvania's CHC program allows certain relatives to become paid caregivers when program requirements are met.

Family Members who may be eligible to provide care include:

  • Adult children
  • Brothers and sisters
  • Other relatives
  • Friends or trusted individuals

Under Pennsylvania's participant-directed services rules, the following generally cannot serve as paid caregivers:

  • The participant's spouse
  • The participant's legal guardian or Power of Attorney
  • Other individuals prohibited by program regulations

All caregivers must meet hiring requirements, complete necessary paperwork, pass required screenings, and comply with program regulations.

How to Apply for Community HealthChoices

Step 1: Apply for Medical Assistance (Medicaid)

If you are not already receiving Pennsylvania Medical Assistance, you must first apply and be approved for Medicaid coverage.

Step 2: Request a Long-Term Services and Supports Assessment

A clinical assessment will be completed to determine whether you meet the nursing facility level of care required for CHC services.

Step 3: Enroll in a CHC Health Plan

Eligible participants select one of Pennsylvania's CHC managed care organizations (MCOs), which will coordinate services and supports.

Step 4: Develop a Person-Centered Care Plan

A Service Coordinator will work with you to identify your needs and develop a customized care plan.

Step 5: Choose Your Home Care Provider

You may select ACS Homecare as your home care agency. If participant-directed services are approved, you may also be able to choose an eligible family member or trusted individual to provide care.
